Educational Module: Spot the Errors-CAUTI and CLABSI Edition

Learn the importance of CAUTI and CLABSI prevention through these immersive activities identifying possible sources of contamination. The accompanying presentations will educate and train the eye to connect common errors to infection risks. Practice these simulated experiences to enrich your facility with knowledge to drive down CAUTI and CLABSI rates.

At the end of this module, participants will be able to:

CAUTI Prevention: Identify common errors in urinary catheter maintenance by analyzing photos that highlight potential sources of contamination.
CLABSI Prevention: Evaluate infection risks associated with central line maintenance through visual identification of common mistakes in simulated clinical scenarios.

The Kentucky Infection Prevention (KyIP) Training Center, located in Louisville, Kentucky, has developed the following content. CAUTI: Spot the Errors Still Image Series

A patient with an indwelling Foley catheter is at risk for a CAUTI. There are practices that should be followed to decrease this risk. Spot the Error

The collection bag is laying on the floor. The floor should always be considered dirty. The collection bag should be secured to the side of the bed. A bag that touches the floor should be considered contaminated.

This photo shows a dependent loop in the tubing. Dependent loops prevent urine from draining to the collection bag. This may also lead to urinary retention despite the catheter being in place.

The urinary catheter tubing is not secured to the patient. Lack of securement may lead to tugging on the internal portion of the catheter, causing trauma and pain, and potentially leading to a urinary tract infection.

The tubing system has become disconnected. The system must remain closed. Any break in the system renders the entire catheter system contaminated, and it must be removed and replaced.

This photo shows catheter tubing that has become kinked. When kinked or twisted, urine is prevented from flowing freely, and may lead to urinary retention, pain, and infection.

Spot the Error

It is important to refrain from allowing the drainage tube to touch the urinal or any other surface. Once the drainage tube has touched another surface, pathogens and other harmful germs have access to the bladder. A contaminated drainage tube renders the entire system contaminated, and should be removed and replaced.
